Dr. Siddappa Gowrav worked in the Great Indian Army as a Captain, field Medical Officer involved in the mountaineering adventure and climbed many mountains in the Great Himalayan Ranges, trained many mountaineers like Bachendri Pal, First Lady to Climb the Mt. Everest. He worked in the rural areas (primary health centers) for a period of 8 years in Rayalseema (A.P.) He was on deputation from Government of India, Ministry of External Affairs to Republic of Maldives for a period of 5 years as a Consultant Anaesthesiologist. As a Professor in Chest Hospital, assisted lot of Thoracic Surgeries to the poor tuberculosis patients. He worked as Cardio-Thoracic Anaesthesiologist in Gandhi Hospital where more than 1200 open heart surgeries were carried out within 26 months. Worked as Superintendent of Niloufer Hospital, expert in Neonatal Anaesthesiology and also Head of Department of Anaesthesia in Osmania Medical College. During the same time, he was principal Osmania Medical College and Additional Director of Medical Education during this period he brought Osmania Medical College to the 14th rank in the Rank Map of Medical Colleges in India.
After retirement in 2011, Government invited him back in the service as Director of Rajiv Gandhi Institute of Medical Sciences (RIMS), Kadapa where he worked for 4 years and streamlined the institution. After relinquishment from RIMS Kadapa, serving in the best and philanthropic private hospital, Malla Reddy Institute of Medical Sciences as Medical Superintendent.
